I, the undersigned, do hereby agree to provide information that will enable Andrea Clark, consultant Trichologist to provide advice and information and where possible treatment recommendations considered necessary in assisting in finding the reasons for hair and scalp conditions. 

I understand any programme or advice provided is not intended to replace or substitute medical advice or care if needed. Any action taken by me as a result of the information received is at my own discretion.

I will continue taking any medication already prescribed by a licensed medical physician and will continue to follow his/her instructions until advised otherwise by them. I agree that I will consult a physician before making any changes.

I agree and understand that Andrea Clark Trichologist is acting on her own behalf and not as an agent of any third party. You agree that services provided by Andrea Clark Trichologist may be provided in premises owned by third parties who have no liability in the provision of any services.
